This form is to be completed in order to be covered by insurance if assisting as a volunteer within your local Little Athletics Centre sactioned events.

This document should be used for all volunteers assisting at the Centre in the case they are not a parent or guardian listed in the registration process of an athlete (parent 1 or parent 2), ordinary members need only complete this form once.
